Every service Alara provides starts from the same place: a real assessment of what this patient actually needs. Our nurses and therapists are trained to see the whole picture, address the whole person, and document everything that matters so nothing falls through the cracks between visits.
Skilled Nursing
Our registered nurses and licensed practical nurses provide skilled nursing care in the home for medically complex patients throughout Las Vegas and Clark County. This is not routine maintenance. This is clinical assessment, disease management, wound care, medication management, and the kind of close monitoring that catches a problem before it becomes a hospitalization.
At every skilled nursing visit, your nurse is asking one question above everything else: is there anything happening here today that will put this patient in the emergency room in the next 72 hours if we don't act on it? That question drives every protocol we have built.
Skilled nursing covers: comprehensive clinical assessment, wound care and dressing changes, IV therapy and medication management, disease management for COPD, CHF, diabetes, cancer, CKD, stroke, and chronic pain, post-surgical care, patient and caregiver education, and care coordination with treating physicians.

Physical Therapy
Our physical therapists work with patients in their own homes — the actual environment where function has to happen. Not a gym. Not a clinic. The living room, the bathroom, the front steps, the specific layout of the specific home where this patient needs to move safely and independently.
We ask every patient the same question at the start of every episode: what do you want to be able to do that you cannot do right now? The answer drives the treatment plan — not a generic protocol.
Physical therapy covers: strength and mobility restoration, balance and fall prevention, gait training, post-stroke and neurological rehabilitation, pain management through therapeutic exercise, home safety assessment, and caregiver training.

Occupational Therapy
Occupational therapy is about getting life back. The ability to shower without exhaustion. To dress independently. To prepare a meal. To do the things that give a day structure and meaning — safely, at home, without depending on someone else for every step.
Occupational therapy covers: activities of daily living training, adaptive equipment assessment, energy conservation, cognitive ADL support, home safety evaluation, caregiver instruction, and upper extremity rehabilitation.

Medical Social Work
This is the service most agencies treat as optional. We treat it as essential.
A nurse can manage a wound. A social worker sees why the wound isn't healing — the patient who hasn't been able to afford adequate nutrition, the caregiver who is one hard week away from breaking, the veteran who hasn't connected with the benefits he earned, the White Card patient who doesn't know he qualifies for more than his card currently covers.
Our medical social workers conduct social determinants of health assessments at every admission, connect patients and families to community resources, assist with advance care planning, and navigate benefits — including EEOICPA consequential conditions, VA compensation, and community assistance programs.
For White Card patients: we screen every patient for depression, malnutrition, anxiety, and chronic pain that may qualify as consequential conditions under EEOICPA — meaning expanded coverage at no additional cost. Most agencies never pursue this. We do it on every admission.

Home Health Aide Services
Our home health aides provide personal care assistance — bathing, grooming, dressing, mobility — under the supervision of a registered nurse. They are in the home more frequently than any other member of the care team, which makes them one of the most important members of it.
At Alara, aides are trained to observe and report. They know what warning signs look like for each patient's specific diagnoses. They know when to call the nurse. And they understand that the person they are helping is not a task list — they are someone trying to hold onto dignity and independence in their own home.
